As we cross the halfway point of the school summer break, preparing for back-to-school in September will be on our minds. Nevertheless, for students who will turn 21 during the school year, it’s also time to plan for graduation next June! The New Jersey Division of Developmental Disabilities (DDD) has just recently released its 2019 timeline Graduates Aging Out of the School System. This timeline outlines the steps to gain access to DDD services and supports they and their families need to take to ensure a smooth transition. This timeline includes the following four steps:
- Ensure Medicaid eligibility.
- Ensure DDD eligibility.
- Complete the New Jersey Comprehensive Assessment Tool (NJ CAT).
- Complete the support agency coordination agency selection form.
These steps are outlined in greater detail on the timeline. Please note that the first three steps can be completed when the student is between 18 and 21 years of age.
